District Service Award- Retired Nomination Form

RETIRED GUIDELINES:

1.   Purpose:   The Northwestern Ohio Education Association has established a NWOEA District Service Award-Retired to be presented to a retired person in recognition of distinguished service to education on the NWOEA district level or having NWOEA district-wide implications SINCE RETIREMENT.

2.   The Award:  The presentation shall be an engraved plaque, designed in an attractive manner.  The number of awards shall be limited to one (1) per year.  The Awardee will be recognized by the NWOEA President (or his/her designee) at the annual NWOEA Representative Assembly unless the Board of Directors determines another appropriate occasion.

3.   Eligibility:   Any member of NWOEA-R may be nominated.  Achievements or contributions meriting the award must be since retirement.  Emphasis should be on criteria for selecting award winners of quality so that this award is remembered as a hallmark of respect and noteworthy accomplishment for the NWOEA district level – not local level.  An individual may be the recipient of this award once only.

Please fill out the form completely.  Incomplete forms will not be considered. This nomination form must be digitally submitted to the NWOEA Center by the 2nd Thursday of January.  Any questions can be emailed to nwoea.center@gmail.com.

Note:  This is a NOMINATION.  It does not guarantee the person will receive an award.  The NWOEA Awards Committee will select award recipients from nominations submitted. The recipients will be notified and invited after the first of February to be recognized at the Awards Reception (More information to come).
