District Service Award- Active Nomination Form

ACTIVE GUIDELINES:

1.   Purpose:   The Northwestern Ohio Education Association has established a NWOEA District Service Award to be presented to a person or a group in recognition for distinguished service to education on the NWOEA district level or having NWOEA district-wide implications.

2.   The Award:   The presentation shall be an engraved plaque, designed in an attractive manner.  The number of awards shall be limited to three (3) per year for active members or groups.  The awardee shall be recognized by the NWOEA President (or his/her designee) at the annual NWOEA Representative Assembly unless the Board of Directors determines another appropriate occasion.

3.   Eligibility:   Any individual, group, or organization, excluding the NWOEA Board of Directors as a group, may be nominated.  (Subdivisions or individuals of NWOEA within the organization may, however, be eligible.)  There is no time limit in achievements or contributions meriting this award.  Award winners should be selected with a particular emphasis on high quality so that this award is remembered as a hallmark of respect and noteworthy accomplishment for the NWOEA district level.  An individual may be the recipient of this award once only.

Note:  This is a NOMINATION.  It does not guarantee the person will receive an award.  The NWOEA Awards Committee will select award recipients from nominations submitted. The recipients will be notified and invited after the first of February to be recognized at the Awards Reception (More information to come).
