Local Service Award Nomination Form
GUIDELINES:
1. Purpose: The Northwestern Ohio Education Association has established a Local Service Award to be presented to a person in recognition for distinguished service to association work on the local level or having local-wide implications.
2. The Award: The Awards Committee shall determine the type of award, to be designed in an attractive manner. THE NUMBER OF AWARDS SHALL BE LIMITED TO THREE (3) PER YEAR PER LOCAL ASSOCIATION. The awardee shall be recognized by the NWOEA President (or his/her designee) at the annual NWOEA Representative Assembly unless the Board of Directors determines another appropriate occasion.
3. Eligibility: Nominations are not limited to one person. There is no time limit in achievements or contributions meriting the award. Emphasis should be on selecting deserving award winners of quality so that this award is remembered as a hallmark of respect and noteworthy accomplishment. Members may receive this award every five years.
Please fill out the form completely. Incomplete forms will not be considered. This nomination form must be submitted to the NWOEA Center by Thursday, January 11, 2024. Any questions can be emailed to nwoea.center@gmail.com.
NOTE: This is a NOMINATION. It does not guarantee the person will receive an award. The NWOEA Awards Committee will select award recipients from nominations submitted. The recipients will be notified and invited after February 1, 2024, to be recognized at the Awards Reception (Findlay Country Club on March 12, 2024 – more to come.)
